"This new survey, released jointly by the Kaiser Commission on Medicaid and the Uninsured with the National Conference of State Legislatures (NCSL), is a comprehensive, detailed database with information about Medicaid benefits states cover, for what populations and the limitations, co-payments and payment methodologies they use. Health Management Associates collected the data in January 2003 using State Plans and State Plan Amendments submitted to the U.S. Department of Health and Human Services₂ Center for Medicare and Medicaid Services (CMS).
